Integrative Healthcare Solutions does not accept all new patient inquiries. We only want to take on patients we feel like we will have the best chance of helping. If you are interested in becoming a new patient then you must complete the steps below to determine if you are eligible to become a patient.

COMPLETE NEW PATIENT FORMS: Download, print, and complete the new patient intake forms ("Adult" for 13 years of age and older, "Child" for 12 years of age and younger). If you can't download/print the paperwork you can contact the office and they can be mailed or faxed to you, (856) 888-1860.

RETURN NEW PATIENT FORMS: Fax, mail, or e-mail (non-secure, at your discretion) the new patient intake forms to the office. If the forms are sent by e-mail they must be in pdf format, we will not accept photos. If you do not have the ability to scan the forms into a pdf, mail or fax them to the office.

REVIEW PERIOD: Once the new patient forms are received in full and reviewed, you will be notified as to whether the doctors feel you are a good candidate to become a patient. You will typically be notified within 3 business days.
If you have not received a response from us within 3 business days then it is likely that we never received your paperwork. Verify the e-mail address (copy and paste from above), check to make sure the fax went through, and/or call the office and verify.

SCHEDULING AND DEPOSIT: Someone from the office will contact you to schedule your new patient exam and take a deposit for the same. The deposit for a new patient exam is $200. The deposit will go towards the full cost ($395) of the new patient exam which is approximately 90 minutes long. Please note that Integrative Healthcare Solutions has a 72 hour cancellation policy (see our Fees and Office Policies in the New Patient Forms). If you cancel your exam with less than 72 hours' notice, your deposit will be forfeited to cover the time set aside for, and the doctors' time spent preparing for the exam. Rescheduling your initial exam will require a new deposit. (Allowances are made for true emergencies.)
